Information For Carers
If you are a carer, or are cared for by a family member or friend, please tell reception so that we can record this information in your medical records.
If you provide, or intend to provide, a substantial amount of care for a relative, friend or neighbour, you have a legal right to ask for a Carer’s Assessment.
A Carer’s Assessment is an opportunity to tell community based services what help you need to continue to provide your caring role.
